Haxtun Health has important information to share with area
Medicaid recipients to ensure they receive the comprehensive care and services they need to improve their health.
Colorado’s Medicaid program is called Health First Colorado. In 2022, 57 percent of all Colorado adults aged 19 to 64 were covered by Health First Colorado. In 2023, eligibility thresholds were changed due to the official end of the COVID-19 Public Health Emergency. Automatic renewals have also stopped. This means fewer Coloradans will be eligible for Health First Colorado. It also means that members are now required to fill out paperwork annually to recertify their eligibility. Those who do not complete the recertification process thoroughly and in a timely fashion will be dropped from Health First Colorado coverage.
To assist area residents with Health First Colorado application and recertification questions as well as to help those who qualify access all their benefits, an organization called Northeast Health Partners was created in 2017. Northeast Health Partners covers Regional Accountability Entity 2 (RAE), which serves the residents of Cheyenne, Kit Carson, Lincoln, Logan, Morgan, Phillips, Sedgwick, Washington, Weld and Yuma counties.
For residents who have Health First Colorado coverage or think they might qualify, the team at Northeast Health Partners is available to help them access: primary and preventive health care, hospital care, prescription medication, dental care, food assistance, transportation, specialty health care, mental health care, substance use treatment, care coordination, housing and more.
